12th Annual Primary Care and Prevention Conference
The 12th Annual Primary Care and Prevention Conference will meet on the sunny and welcoming island of San Juan, Puerto Rico, Sept. 27-28, 2012 to convene the leading authorities in health care and public health to examine, “Best Practices for Planning the Future of the Patient Centered Medical Home.”
The two-day hands-on conference will be designed to offer attendees applicable strategies and tools to begin the implementation of a Patient Centered Medical Home or further their current efforts toward this gold standard of care. Participants will be engaged by experts who are well versed on the pitfalls and challenges of the Patient Centered Medical Home and the best practices associated with this model of care.
Attendees will receive tools and templates pulled directly from our Patient Centered Medical Home best practices as part of the hands-on experience. Conference speakers will vary, representing both the private and government sectors and the profit and non-profit sectors.
