You must complete the Title IV Funds Authorization Form if you are applying for financial aid in the 2012 - 2013 school year. Students must place their Banner ID#/M# on documents submitted for review in the 2012-2013 academic term.
Written documentation must be provided to verify that you filed the FAFSA or renewal FAFSA. The FAFSA must be completed by the financial aid deadline dates as outlined on the Application Deadlines webpage.
Errors will delay the processing of your financial aid information. We recommend that you complete the 2011 tax return prior to completing the FAFSA form.
Federal Income Tax Return: You must forward signed and dated copies of your, your spouse's (if married), and your parents'* completed 2011 Federal Income Tax Return, W-2 forms, 1099 forms, and tax schedules.
*Only applies to applicants seeking institutional funds for the M.D. and M.P.H. Programs.
- Non-Filer: If any of the persons listed above are required to submit tax returns, have neither filed nor intend to file a 2011 Federal Income Tax Return, they must complete a "Non-filers Certification Form" (W-2 forms and 1099 forms must be submitted to verify all taxable and untaxable earnings, interest income payments, annuities, etc.).

- Federal Tax Return Not Complete: If you, your spouse, and/or your parents have not completed a 2011 U.S. income tax return but will be filing one, it is recommended that you complete your tax return before filling out the appropriate need analysis form. Knowing what will be on your tax return will help you complete the need analysis form accurately.
When your need analysis form is compared with the official 2011 IRS Form 1040, or 1040EZ that is filed by you and your family, the financial information must agree. If there are any errors, this will delay your financial aid process and it will take longer to determine your eligibility and receive institutional and federal student aid.
Important: Federal Direct loan applications cannot be processed until all information on the FAFSA form is correct.
NOTE: Do not complete the Graduate Plus loan application and credit check prior to June 13, 2012. Credit checks for this loan expire within 90 days after completion. Submission of any additional credit checks could result in a denial if adverse changes exist on your credit report. Current students may complete loan applications/credit checks June 14, 2012 through June 28, 2012.
The Parent Section of the FAFSA Form must be completed (MD and MPH students only) if you wish to be considered for institutional financial aid (i.e., MSM grants, MSM loans, SDS, LDS, Primary Care Loans).
Note: STUDENTS WHO DO NOT SUBMIT THE REQUIRED INFORMATION BY THE ESTABLISHED DEADLINE DATES WILL NOT RECEIVE PRIORITY CONSIDERATION FOR INSTITUTIONAL FUNDS.
Eligibility Requirements
To qualify for most financial aid programs and to maintain eligibility for these programs, federal regulations state you must meet the following qualifications:
- U.S. Citizen or permanent resident
- Accepted for admission to the medical school
- Enrolled in good standing at least half-time
- Demonstrate financial need. Financial need is the difference between the reasonable and approved costs incurred by a student at MSM and all resources available to the student (i.e., contribution from the parents, student and spouse (if applicable), and any outside aid the student is eligible to receive).
- Maintain satisfactory academic progress for financial aid.
- Registered for the draft with Selective Service if you are a male, at least 18 years old, born after January 1, 1960, and not currently a member of the Armed Forces.
- Once the above criteria are met, funds are released only if the student has no debts outstanding to MSM and has not defaulted on any Title IV Higher Education Act (HEA) loan, (Federal Subsidized and Unsubsidized Stafford Loan, Federal Perkins, Federal PLUS/Federal SLS) made for attendance at any institution, and does not owe a refund on any Title IV HEA grant (Federal Work Study, Federal SEOG, Federal Pell Grant, SSIG) received for attendance at any institution.
