Thank you for agreeing to participate in the Dr. Daniel S. Blumenthal Public Health Conference scheduled for Wednesday, April 4, 2018. The Conference will take place at the Louis W. Sullivan National Center for Primary Care at Morehouse School of Medicine located at 720 Westview Drive, Atlanta, GA 30310. As we approach the convening of the conference, I am pleased to submit to you a few items to assist in preparing you for your participation.

Audio Visual: We will provide LCD projector and laptop |

Presentation: Please submit PowerPoint presentations in PC compatible format no later than March 1, 2018. Mac platforms will not be available. |

Biographical Sketch and Head Shot: Please submit a copy of your biographical sketch and head shot electronically to ngreen-harris@msm.edu. by March 1. Please ensure photo is in jpg, png, gif, or eps format. The bio sketch will be used to introduce you and should not exceed 200 words. |

Travel Expenses: We will cover basic travel related expenses (meals, parking, ground transportation, air and hotel) if required. A staff member from MSM Department of Community Heath and Preventive Medicine will contact you to assist with travel arrangements. Most expenses, with the exception of airfare and hotel will be reimbursement. |

Disclosure of Financial Relationship: This is an accredited CME program which requires all presenters to disclose any potential conflicts of interest to Morehouse School of Medicine Office of Continuing Medical Education. Refusal to disclose any conflicts of interest will disqualify you from participation in the event. Please complete the disclosure form using the link CE Disclosure no later than March 1, 2018. |
